Forms must be filled in completely.
Even if you have sent me entries for the last 8 years for the same dog, please don't omit any required information.
Forms must be legible.
If I can't read it, it is not a valid entry and will be returned. (I will send you a form filled out with your dog's
information if requested.)
Send a copy of your dog's height card.
Every time, no exceptions. It is not kept on file.
Always bring your card to the trial.(Please cut the copy down to card size and don't send a full sheet of paper.)
Check your math!
Entries without sufficient fees paid will be returned.
The above are AKC requirements. Below are a couple of requests I'd like to make.
NO STAPLES, PLEASE!
Everything can be loose in the envelope or paper clipped together. Height card copy may be taped to the entry form.
Make sure my email address is added to your 'OK' list. If you send an email address to me, I assume that is how
you want to receive confirmations, notices and be contacted.
